Standard #: 22VAC40-185-130-B Description: Based on record review and interview, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The most recent immunizations on file for child 1, age 13 months, were administered on 06/11/2019.
a. Child 1 was in care on 05/01/2020.
b. The program director confirmed that updated immunizations were not on file for child 1 as of 05/05/2020 when the record was reviewed.Plan of Correction: All current children's files will be audited and corrected by 05/15/2020.
Going forward all families will have a paperwork appointment or file audit prior to returning into care to ensure all files are complete.
