An unannounced monitoring inspection was initiated on 03/13/2020 in response to a self-reported incident regarding toileting. The inspector reviewed records, interviewed one staff and reviewed the physical plant on this date.
This inspection was completed by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. Six staff interviews were conducted by telephone on 04/24/2020, 04/28/2020 and 04/29/2020.
Based on the information gathered this allegation is found to be not valid. No violations were issued.
