The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated May 8, 2020.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. The Licensing Inspector was not on site for today's inspection.
A renewal inspection was initiated on May 7, 2020 and concluded on May 8, 2020. The director person was contacted by telephone to initiate the inspection. There were no children present and no staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records, and background checks for board members submitted by the facility to ensure documentation was complete.
The child day center has been closed due to the emergency health pandemic since March 12, 2020.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-191-40-D-1-A Description: Based on record review and provider statement, one agent of the child day center did not have a completed sworn statement or affirmation and the results of a central registry check before the end of 30 days of assuming the a position of a principal officer.
