Comments:
An unannounced investigation was conducted 02/04/2020 in response to a complaint regarding staff to child ratio for two year old children. During the investigation staff were interviewed, the files of children and staff were reviewed, and the two's 1 and two's 2 rooms were observed. The preponderance of evidence supports the allegations.This complaint has been determined to be valid.The exit interview was conducted with the director. Complaint related: Yes
Description: Based on interview, records and reports on children were not maintained and made accessible for two years after termination of services.

Evidence:
1. Staff A could not produce past name to face records.
2. When asked for all name to face records for 01/23/2020, 01/24/2020 and 01/27/2020, Staff A stated that she could not find them.

Plan of Correction: Other Attendance records are available for viewing if FTN sheets are not available if needed. A system is in place to turn in FTN sheets daily.

Complaint related: Yes
Description: Based on record review and interview, for two-year-old children: one staff member for every eight children were not kept.

Evidence:
1. On 01/23/2020 and 02/04/2020 the ratio of one staff for every group of eight two-year-old children was not kept.
2. Staff B confirmed that on 02/04/2020 she was left alone with 10 2 year olds in her care.
3. Multiple staff interviews and the ?Face-to-Name Transition Sheet? dated 01/23/2020 for the Early Preschool classroom confirmed that staff-to-children ratio requirements were not met from approximately 8:12 am to 9:00 am, when 11 children were under the supervision of 1 staff member, exceeding the ratio by 3 children.

Plan of Correction: Teacher was in ratio but was not indicated on the FTN sheet. Director was in classroom on 02/04/2020 in ratio. All teachers are retrained on FTN procedures. Staff to child ratios will be maintained at all times.

Description: Based on interview, the center reassigned children from their regular group and staff members casually and repeatedly.

Evidence:
1. Staff C stated children are moved to accommodate ratio.
2. Staff D stated children are moved around in the morning to accommodate ratio.
3. Multiple staff interviews confirmed that staff-to-children ratio requirements are not met in the mornings, when classrooms are combined, for approximately 2-5 minutes until a manager is available to assist and/or move children around to other classrooms at least once or twice a week.

Plan of Correction: Schedules were adjusted to maintain a more stable environment in the mornings. Maintain schedules as much as possible to prevent in future.
