This inspection was conducted by licensing staff using an alternate removte protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on May 8th, 2020 and concluded on May 8th, 2020. The provider was contacted by telephone t initiate the inspection. There 9 children present and 5 staff members. The inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure the documentation was complete.
Information gathered during the inspection determined non compliance with the applicable standards or law and violations were documented o the violation notice issued to the facility.

Evidence:
Child #1's records did not contain documentation of work phone number and place of employment of each parent who has custody.Plan of Correction: I will get information from parent and place in child file.
