This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 05/08/2020 and concluded on 05/12/2020. The program director was contacted by telephone to initiate the inspection. There were no children in care as the facility is currently not operating due to the pandemic. The inspector emailed the program director a list of items required to complete the inspection. The inspector review 2 children's records and 2 staff records during the inspection to ensure documentation was complete. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-60-A Description: REPEAT VIOLATION
Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child #2 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the address for the second designated contact was incomplete.Plan of Correction: The provider will get the missing information from the parent as soon as possible.
Standard #: 22VAC40-185-70-A Description: REPEAT VIOLATION
Based on record review and discussion with the staff, the facility failed to ensure that staff records contain all of the elements as required by the standards.
Evidence:
1. The record for Staff #1 did not contain written information to demonstrate that the individual possesses the orientation training required by the job position; the documented hire date for Staff #1 was 01/23/2020.
2. The record for Staff #2 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment. The record only contained one reference check and the documented hire date for Staff #2 was 11/05/2018.Plan of Correction: The provider will request the missing information from the staff and add it to the records.
