The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
- Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/14/2020 and concluded on the same date. The director was contacted by telephone to initiate the inspection. There were no children present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-185-60-A Description: Based on record review and interview, one out of two children's records reviewed did not contain all required information.
Evidence:
1) The record for Child #1 did not contain documentation of viewing proof of the child's identity and age.
2) The facility representative stated that they did not ask for proof of the child's identity.Plan of Correction: I will ask the parents for the documentation when we return to school.
