This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 4/28/2020 and concluded on 4/28/2020 and 5/1/2020. A self-reported incident was received by the department regarding allegations in the areas of forbidden action. The director was contacted by telephone to conduct the investigation.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-410-1 Description: Based on interviews with staff, the center failed to ensure that physical punishment, striking a child, roughly handling or shaking a child was not used as a form of punishment.
Evidence:
1. Staff # 1 stated that child # 1 was running up to her and slapping her leg in a playful manner. Child # 1 did this several times. Staff # 1 took child # 1 and placed the child over her lap and playful spanked the child's bottom. Staff # 1 stated the child was laughing and did not cry.Plan of Correction: Not available online.
