The provider has stated that they didn't think they were a subsidy provider and that there are currently no subsidy families enrolled at the program. If the provider does not want to be a subsidy vendor, please contact subsidy at 833-551-7794 to terminate your contract.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated and concluded on 5/22/2020. The Director was contacted by telephone to initiate the inspection. There were not children present during the inspection. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-540-A Description: Based on record review and interview, staff did not submit documentation of a tuberculosis test/screening that had been completed within the last 30 calendar days prior to the first date of employment.
Evidence: The TB screening submitted by Staff #1 (date of hire: 2/10/2020) is dated 6/12/2019.Plan of Correction: Staff will receive an updated TB Screening as soon as it is safe to do so.
