Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on May 4, 2020 and concluded on May 5, 2020. The director was contacted by telephone to initiate the inspection. There were no children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center did not ensure before a child may attend the center, the center shall obtain documentation that the child has been adequately immunized.
Evidence: 1. The record of Child #1 (DOE 4/3/18) contained documentation of immunizations dated 4/6/18.
2. Administration stated the center had the immunizations within one week of the child's date of enrollment.

Plan of Correction: Central will determine that parents will turn in all immunization records on or before their child's start date

Standard #: 22VAC40-191-60-B
Description: Based on record review and interview, the center did not ensure all employees completed a sworn disclosure statement prior to employment.
Evidence: 1. The record of Staff #1 (DOH 12/9/19) contained documentation of a sworn disclosure statement dated 2/25/20.
2. Administration stated Staff #1 had another sworn disclosure statement that was not signed or dated because the document was not centered correctly and no signature/date line showed on the document and that her second sworn disclosure statement was signed 10 days after her start date.

Plan of Correction: Central will determine and document that all sworn statements are signed and dated and completed before a staff member begins to work at Central.
