This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/19/20 and concluded on 5/22/20. The director was contacted by telephone to initiate the inspection. There were 7 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The record for child 2 did not contain documentation of a physical examination by or under the direction of a physician.Plan of Correction: I will let mother know of missing last page. I will ask her to contact her doctor to set appointment and have her records here by 7/1/2020.
Standard #: 22VAC40-185-70-A Description: Based on record review, one staff file was not maintained.
Evidence:
