This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on Thursday, June 4, 2020 and concluded on Friday, June 5, 2020. The director person was contacted by telephone to initiate the inspection. There were 45 children present and 10 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #: 22VAC40-185-60-A Description: Based on review of children's record, the center did not ensure to obtain all information for children's records as required.
Evidence: In Child#1's, Child#3's, and Child#4's record, the addresses for the emergency contact information are incomplete.Plan of Correction: The Director will reach out to parents to update emergency contacts in the system for each child.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of record, documentation, and interview with staff, the center did not ensure to obtain an Office of Background Investigation (OBI) determination letter results prior to employment of staff.
Evidence: In Staff#3's record (date of employment 12/17/2019) the OBI determination letter was dated 01/07/2020. Documentation of the staff orientation training was dated 12/07/2019. Interview with staff confirmed the staff's date of employment and completion of orientation training 12/17/2019 prior to results obtained.Plan of Correction: The Director will contact the center's hiring specialist to ensure the center is not hiring or completing orientation training with staff before the OBI fingerprinting results are received by the center.
