Technical Assistance:

All required background checks need to be completed. Documentation of submission needs to be sent to inspector within 10 business days (6/23/20). Fire inspection expired in April due to suspension and now backlog of Fairfax Fire Department inspection.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on June 8, 2020 and concluded on June 9, 2020 The director was contacted by telephone to initiate the inspection. The facility is currently closed and will reopen on June 24, 2020. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-191-40-D-1-A
Description:	Based on staff interviews and review of staff records, background checks were not obtained for a new person designated as an applicant. Evidence: There is no documentation of a signed sworn statement or submission of a central registry background check for staff members #1,2,3.
Plan of Correction:	A request for documentation of signed Sworn Statements and Central Registry background checks has been submitted to home office. Principal will send inspector documentation when received.
Standard #:	63.2(17)-1721.1-B-2
Description:	Based on staff interviews and review of staff records, fingerprint checks were not obtained for a new person designated as applicant. Evidence: There is no documentation of a eligible fingerprint letter for staff members #1,2,3. Principal will send inspector documentation when received.
Plan of Correction:	A request for documentation of eligible letters for a fingerprint background check has been submitted to home office. Principal will send inspector documentation of submission within 10 business days and final results when received.

Description:	Based on staff interviews and review of staff records, out of state background checks were not obtained for a new person designated as applicant. Evidence: There is no documentation of submission of an out of state central registry background check for staff members who have lived out of Virginia within the past five years for staff members #1,2,3. Principal will send inspector documentation when received.
Plan of Correction:	A request for documentation for out of state CPS background checks for staff who have lived out of the commonwealth of Virginia within the past five years has been submitted to home office. Principal will send inspector documentation of submission within 10 business days and final results when received.
