This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 6/4/2020 and concluded on 6/5/2020. The director was contacted by telephone to initiate the inspection. There were 0 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain documentation of a physical for one child within one month after attendance.
Evidence:
Child #1 began attendance on 1/16/2020. The record for Child #1 did not contain documentation of a physical exam.Plan of Correction: Director contacted Child #1's parent to request a copy of the physical. The parent has reached out to the physician's office and will forward the physical once received.
Standard #: 22VAC40-185-60-A Description: Based on record review, 2 of 2 children's records did not contain all required information.
Evidence:
The records for Child #1 and Child #2 did not contain the work number for the child's father.Plan of Correction: The information has been requested from parents. I will request the information again and update the form to include a work number specifically.
