Comments:
An unannounced monitoring inspection was completed on 6/8/2020. The inspector and the LA arrived at 10 A.M. and departed the program at 12:00 P.M. There were 19 children in care with five staff present. Two children's records and three staff records were reviewed.

Observed the children during: feeding time, free play, and table toys time.

Violations:
Standard #: 22VAC40-185-140-A
Description: REPEAT VIOLATION

Based on review of two children's records, the center failed to ensure that all children enrolled after 30 days had a physical examination.

Evidence:

1. Child # 2 started on 7/16/2019 and on the day of the inspection did not have a physical examination.

Plan of Correction: The director will have the parent to obtain a physical and all records will be monitored.

Description: REPEAT VIOLATION

Based on review of two children's records, the center failed to ensure that all records were complete.

Evidence:
1. Child # 1 was missing phone number for one of the two required emergency contacts. Child # 1 was also missing allergies on the enrollment form.

Plan of Correction: The director will make sure that all records are complete and will monitor.

Standard #: 22VAC40-185-70-A
Description: Based on review of three staff's records, the center failed to ensure that all staff had a record.

Evidence:
1. Staff # 3's record could not be located during the inspection.

Plan of Correction: The director stated that she had the record recently and someone cleaned up and she can not locate the record. The center will ensure that all records are stored in a manner that they can be located at any given time.

Standard #: 22VAC40-185-210-A
Description: Based on review of two staff's records, the center failed to ensure that all acting program lead teacher had the required qualifications.

Evidence:
1. Staff # 1 and 2 did not have a high school diploma or equivalent, and the required education component for a program lead.

Plan of Correction: The director will work on obtaining the missing components and training required for the position.

Standard #: 22VAC40-185-240-A
Description: Based on review of two staff's records, the center failed to ensure that all staff had orientation training by the end of their first day of assuming job responsibilities.

Evidence:
1. Staff # 2 was hired on 10/25/2019 and there was no orientation in the staff's record.

Plan of Correction: The director will make sure that the orientation was completed and in the staff's record.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that all food items brought from home met the requirements per the standard.

Evidence:
1. There were four lunch boxes that were not labeled with the owners name. All the food brought from home were not dated.

Plan of Correction: In the future all lunches will be labeled and dated every day.
