This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A (monitoring/renewal) inspection was initiated on 06/08/2020 and concluded on 06/11/2020. The director was contacted by telephone to initiate the inspection. There were 43 children present and 11 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-60-A Description: Based on a record review, the center did not ensure all required information was maintained in each child's record.
Evidence:
Child #2 (enrolled 01/20/20) did not have two complete emergency contact information, as required.Plan of Correction: This was corrected on 6/11/20. Going forward parents are required to have a complete paperwork appointment to ensure a 100% completed file signed off by two managers prior to start. All current files of active children will be checked for compliance in 30 days. DM will conduct audits on routine inspections.
Standard #: 22VAC40-185-260-B Description: Based on a record review, the center did not provide annual approval from the health department.
Evidence:
The health report provided by the director was not dated.Plan of Correction: Corrected on 06/12/2020. Moving forward management will check for completeness when signing off on all inspections.
