Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 05/21/2020 and concluded on the same date. The director was contacted by telephone to initiate the inspection. There were no children present and 2 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-90--A
Description: Based on record review and staff interview, it was determined that one of two records reviewed did not contain a written agreement, signed by the parent, between the parent and the center that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.
Evidence:
1) The record for Child #2 did not contain a statement that the parent will inform the center within 24 hours of exposure to any communicable disease.
2) The Director could not locate the statement in the file.

Plan of Correction: We will add the statement to the child's record and ask the parents to sign it once we reopen the center. It will be included in the child's record.
