The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on June 4, 2020. The director was contacted by telephone to initiate the inspection. There were no children due to the center being closed since March. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed (5) children?s records and (5) new staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with two applicable standards and were documented on a violation notice issued to the facility.

Standard #: 22VAC40-185-130-B Description: Based on review, 2 of 5 children files indicated that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
1. On the date of inspection (06/04/2020), the following immunization pages were observed: For Child #2 (immunization page dated - 07/23/2019); and for Child #3 (immunization page dated - 09/05/2019).
2. Based on information on file, both Child #2 and Child #3 were under the age of 2 years on the date of inspection.
Plan of Correction: Updated documentation shall be obtained for both children and placed on file.
Standard #: 22VAC40-185-140-A Description: Based on review, 1 of 5 children files indicated that each child did not have a physical examination (by or under the direction of a physician) within one month after attendance.
1. On the date of inspection (06/04/2020), it was determined that documentation of a physical examination for Child #5 was not available for review.
2. The start date for Child #5 was listed as 10/28/2019.
Plan of Correction: The parent of Child #5 shall be notified and documentation of a physical obtained and placed on file.
