Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on June 12, 2020 and concluded on June 12, 2020. The director was contacted by telephone to initiate the inspection. There were 12 children present and three staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain the required information.
Evidence:
1. Child #1's record did not contain documentation of the names of the persons authorized to pick up the child.

Plan of Correction: Director will have the parent to document the record with the names authorized to pick up the child.

Standard #: 22VAC40-185-60-A-7
Description: Based on record review, children's record did not contain documentation of any allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation.
Evidence:
1. Child #2's record did not contain information regarding allergies, intolerance to food, medication, or any other medication, or any other substances, and actions to take in an emergency situation. This information was left blank on the record.

Plan of Correction: I will have the family to document the record.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review and interview, the center did not request annual parent confirmation that the required information in the child's record was up to date.
Evidence:
1. Child #1 and Child #2's record did not contain documentation of an annual review.
2. The Director stated that documentation was not in Child #1 and Child #2's record.

Plan of Correction: Documentation of an annual update will be completed by both families and placed in the record.
