This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 6/12/20 and concluded on 6/17/20. The center owner was contacted by telephone to initiate the inspection. There were 9 children present and 3 staff. The inspector emailed the center owner a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.
Evidence:
1. The record for child 1 (date of enrollment: 9/8/17) does not contain documentation of a physical examination.
2. The center owner confirmed that the record for child 1 is lacking documentation of a physical.Plan of Correction: The center responded with the following: The parent of child 1 has been contacted and will have the doctor fax a copy of the child's most recent physical to the center.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and interview, the center did not ensure that staff who have lived outside of the state of Virginia within the past 5 years obtain a central registry check from that state.
Evidence:
1. Staff 1 (date of hire: 4/1/20) indicted on her sworn statement or affirmation that she has resided in the state of North Carolina within the past 5 years. The record for staff 1 does not contain the results of an out-of-state central registry search for the state of North Carolina.
2. The center owner confirmed that the record for staff 1 does not contain the results of an out-of-state central registry search for the state of North Carolina.Plan of Correction: The center responded with the following: The out-of-state central registry request for North Carolina will be completed for staff 1 and mailed on 6/18/20. Going forward, center management will double check the sworn statements of new hire's to ensure that out-of-state central registry searches are requested, when applicable.
