Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on June 22, 2020 and concluded on June 23, 2020. A complaint incident was received by the department on March 3, 2020, regarding allegations in the areas of procedures for emergencies. The director was contacted by telephone to conduct the investigation. The inspector requested a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Complaint related: Yes
Description: Based on a review of records on June 22, 2020, the center did not ensure to obtain all of the information of written record of children's serious or minor injuries in which entries are made the day of occurrence to include staff and parent signatures or two staff signatures.
Evidence: 1. The accident report for Child #1 contained documentation of one staff signature dated 3/2/2020.
2. Administration acknowledged on the accident report there was a delay. Next to the administrator's signature it states "sorry for the delay" and is dated 3/6/2020.

Plan of Correction: Remind all staff of our incident/accident reporting procedures (staff training) - also update our reporting policy in the operating manual, requiring reports to be fully completed on the day of occurrence.
