Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on a review of records on June 22, 2020, the center did not ensure to obtain all of the information of written record of children's serious or minor injuries in which entries are made the day of occurrence to include staff and parent signatures or two staff signatures.
Evidence: 1. The accident report for Child #1 contained documentation of one staff signature dated 3/2/2020.
2. Administration acknowledged on the accident report there was a delay. Next to the administrator's signature it states "sorry for the delay" and is dated 3/6/2020.
Plan of Correction: Remind all staff of our incident/accident reporting procedures (staff training) - also update our reporting policy in the operating manual, requiring reports to be fully completed on the day of occurrence.

In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights.
