One of more background checks were found to be missing during the inspection of the program. The Director must provide documentation that the missing background checks have been initiated to the licensing inspector within 10 business days of the inspection.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 6/10/2020 and concluded on 6/15/2020. The Director was contacted by telephone to initiate the inspection. There were no children present with the Director. The inspector emailed the Director a list of items required to complete the inspection. The inspector reviewed two children's record and two staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-140-A Description: Based on record review, the center has not obtained a physical exam for each child within the first 30 days of attendance.
Evidence: Child #1 (date of enrollment: 7/18/2018) record contained a blank physical exam.Plan of Correction: I will email the parents today and request that they bring the missing paperwork when the child returns.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all required information.
Evidence: Child #1 (date of enrollment: 7/18/2018) record did not contain documentation that the child's age and identity has been verified within the first 7 days of attendance or documentation of an annual review of emergency contact information.Plan of Correction: I will email the parents today and request that they bring the missing paperwork when the child returns.
Standard #: 22VAC40-191-40-D-1-A Description: Based on record review, signed sworn disclosure statements and a submission of a central registry background check were not obtained for a new person designated as officers of the corporation.
Evidence:
1. Sworn disclosure statements have not been obtained for Board Officer #2.
2. Central registry background checks have not been obtained for Board Officer #1 and Board Officer #2.Plan of Correction: I will make contact with our corporate office as soon as possible.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on record review, criminal history records (fingerprints) have not been obtained for new persons designated as officers of the corporation.
Evidence:
1. Fingerprint results have not been obtained for Board Officer #2.Plan of Correction: I will make contact with our corporate office as soon as possible.
Standard #: 63.2(17)-1721.1-B-3 Description: Based on record review, out-of-state central registry background checks have not been obtained for new person designated as officers of the corporation whom reside outside of the state of Virginia.
Evidence:
1. Board Officer #1 resides in California and an out-of-state central registry background check has not been initiated.
2. Board Officer #2 resides in Pennsylvania and an out-of-state central registry background check has not been initiated.Plan of Correction: I will make contact with our corporate office as soon as possible.
