Technical Assistance:
Inspector had a discussion with center management regarding potentially needing to alter their procedures for getting school aged children out to the buses safely.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on 3/20/2020 and concluded on 6/22/2020. A complaint was received by the department regarding allegations in the areas of supervision. The Director was contacted by telephone to conduct the investigation. The inspector requested a list of documentation required to complete the investigation and staff were interviewed.

The evidence gathered during the investigation supports the allegation of non-compliance with standards or law and violations were issued. Any violations identified during the investigation

Description: Based on interview and record review, staff could not provide immediate intervention when needed.
Evidence:
1. On 3/12/2020 at approximately 8:25am, Staff #1 was taking 12 children out of the facility to get onto the programs school bus to go to the local elementary schools.
2. Staff #1 stated that about half of the children ran out of the programs front door approximately 30 feet away to the bus while they had to stay at the front door to get the rest of the children that were still in the program lobby.
3. Child #1 picked up a piece of sharp plastic off the ground near the bus and held it to Child #2's neck and stated "I'm going to kill you". Child #2 tried to push Child #1's hand away and the plastic cut Child #2's cheek.
4. Staff #1 was not able to intervene the incident between Child #1 and Child #2 because they were at the school front door trying to get the rest of the group of children outside.

Plan of Correction: Buses will be started by management before children leave the center. Bus drivers will only walk kids out who are on their buses in a single file, walking line. Drivers will be at bus doors making sure students get on safe before getting on bus.
