An unannounced Subsidy Health and Safety (SHSI) monitoring inspection was conducted on March 4, 2020 from 9:05am to 3:08pm to determine the center?s compliance with licensing standards. The children were observed making crafts, playing puzzles, rhyming in circle time, and having snack. The licensing inspector interacted with staff and children when appropriate. All classrooms, playgrounds, and buses were inspected today.
Nine children?s records and five staff records were reviewed during this inspection.

The center administers medication. Medications and authorizations were reviewed and found complete. Injury reports were reviewed.

The center's first aid kit and non-medical emergency supplies were inspected and found complete.

The director was available for the inspection. The director was present at the exit interview at which time inspection findings were reviewed and an Acknowledgment of Inspection form was signed and left with the licensee.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Two violations of the supplemental subsidy health and safety requirements were found.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the vendor did not ensure that two of five staff submitted documentation of the tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days.
Evidence: 1. The record of Staff #1 (DOH 8/1/19) contained documentation of a tuberculosis test dated 11/13/18.
2. The record of Staff #3 (DOH 11/25/19) contained documentation of a tuberculosis test dated 1/22/19.

Description: Based on record review, the provider did not ensure within the first 90 days of employment of service all caregivers complete Virginia Pre-Service Training for Childcare Staff, sponsored by the Department of Social Services.
