Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 6/24/20 and concluded on 6/25/20. The person in-charge was contacted by telephone to initiate the inspection. There were 14 children present and five staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 7/3/2020. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of two children's records, the center did not obtain all required documentation for a child's record. Evidence: The record for Child A did not contain documentation of either parent's work phone number or the place of employment for one parent.

Plan of Correction: Information was updated.

Standard #: 22VAC40-185-240-D-4
Description: Based on staff interview, a child for whom emergency medications have been prescribed was not always in the care of a staff member who has received training in medication administration. Evidence: 1. There is an epi-pen on site for Child B, with a dairy allergy, but only one staff member has Medication Administration Training (MAT) and that staff member is out of the building for the week of June 22-26, 2020.

Plan of Correction: MAT renewal for Director and MAT training for other staff has been scheduled for 7/7/20. We will attempt to bring back another staff who has the training until then.
