This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on Tuesday, June 23, 2020 and concluded on Tuesday, June 30, 2020. The director was contacted by telephone to initiate the inspection. There were no children and staff present during the remote inspection as the program is currently closed. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children records submitted by the facility to ensure documentation was complete. There was an annual staff record review conducted in 01/2020. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #: 22VAC40-185-140-A Description: Based on review of children's record and interview with staff, the center did not ensure all obtain physical documentation for all children within one month after attendance.
Evidence: In Child#2's record (start date 09/04/2020), there was no documentation of a physical found. Interview with staff confirmed the physical for Child#2 was not available for review during the inspection.Plan of Correction: The timeline needs to be kept for obtaining the physical documentation from the parent. The program will follow up to ensure a copy is included in records for review.
