This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on July 9th, 2020. The director was contacted by telephone to initiate the inspection. There were 6 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-130-A Description: Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
Child #2's date of attendance was 2/28/2020 the immunizations were dated 5/10/19.Plan of Correction: We will obtain documentation of immunizations on the first day of attendance when children enroll.
Standard #: 22VAC40-185-140-A Description: Based on a review of children's records, the center failed to obtain documentation of a physical by the 30th day of attendance.
Evidence:
Child #'2's first date of attendance was 2/28/2019, the date on the child's physical is 5/10/19.Plan of Correction: We will obtain documentation of a child's physical by the 30th day of attendance of a newly enrolled child.
