Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on July 15th, 2020. The assistant director was contacted by telephone to initiate the inspection. There were 15 children present with 4 staff. The inspector emailed the assistant director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the Violation Notice issued to the facility.

Violations:
Standard #: 22VAC40-185-150-A
Description: Based on a review of children's records, the center failed to obtain the current form required by the Virginia Department of Health used to report immunizations and the results of physical examination.

Evidence:
Child #2's record did not have documentation of a physical on the current form required by the Virginia Department of Health.

Plan of Correction: We will require parents to provider the correct Virginia Heath Form.

Standard #: 22VAC40-185-550-D
Description: Based on a review of administrative records,the center failed to implement a monthly practice evacuation drill.

Evidence:
The center did not have documentation of a practice evacuation drill for the month of June 2020.

Plan of Correction: We will conduct monthly practice evaluation drills.
