Technical Assistance:
1. Areas of the home that are not clean and safe should not be used by children in care. The back deck has indoor furniture stacked there, one of which is a sofa standing on end just outside the glass door. Household items inside should be stored in a way that it is safe as well. 2. New regulations document given to vendor who will seek assistance from family members to read/review. 3. Forms provided: attendance, new emergency plan, and proof of identity.

Comments:
12:15-1:40pm Annual unannounced subsidy inspection completed. Observed 4 children = 9 points with the vendor. Ratios and supervision were in compliance. Children were napping, coloring, and playing with blocks and a train. The vendor was speaking to the children in Spanish, and promptly tending to their needs. Toys and equipment for the children were sufficient. The vendor is currently trained in First Aid/CPR. Recordkeeping non-compliances found. Mobile translation was used for communication between vendor and inspector. The vendor will seek assistance from family members to read/review this report.

Description: Based on records reviewed, it was determined that 3 of 4 children's records did not include required information. Evidence: 1. Child #4 - Name, address + phone number for an emergency contact person, other than parent. 2. Child #5 - Name, address + phone number for an emergency contact person, other than parent. 3. Child #6 - Home/work address and phone number for 1 of 2 parents listed; address + phone number for emergency contact person listed.

Plan of Correction: Parents will be asked to complete the paperwork.
