Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on July 27th, 2020. The director was contacted by telephone to initiate the inspection. There were 27 children present with 12 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children's records and 3 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance wit the applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records, the center failed to obtain documentation of immunizations by the first day of attendance.

Evidence:
Child #3's record did not have documentation of child's immunizations on the first day of attendance. Child's first day was 7/13/2020, immunization documentation was dated 7/14/2020.

Plan of Correction: I will ensure that all children will have proof of immunizations on the first day of attendance.

Standard #: 22VAC40-185-90--A
Description: Based on a review of children's records, the center failed to obtain a written agreement between the parent and the center, signed by the parent to include a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Evidence:
Child #1 and #3's records do not have documentation of a statement that the center will notify the parent when the child becomes ill and the parent will arrange to have the child picked up as soon as possible or a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable diseases.

Plan of Correction: I will have the parents complete all required parental agreements.
