Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on August 6, 2020 and concluded on August 6, 2020. The director was contacted by telephone to initiate the inspection at 11:40 a.m. A virtual inspection was conducted beginning at 12:20 p.m. Walk-through of the buildings were conducted. There were six classrooms: infants, toddlers, two's, three's, pre-k, and school age in operation today. There were a total of 46 children in care ranging in age from two months to nine years of age under the care and supervision of 6 staff. Infants were observed sleeping, being held and having playtime. Toddlers, two year olds, three year olds and prek was observed preparing for nap or napping. School agers were observed during lunch. Lunch today was chicken salad, crackers, celery sticks, watermelon and milk. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children?s records and four staff records submitted by the facility to ensure documentation was complete. Staff training hours were reviewed. Emergency drills are up to date.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, the center did not have documentation of immunizations by the first day of attendance. Evidence: Child #3, enrolled 10/17/2019, did not have documentaion of immunizations in the record.

Plan of Correction: Child#3 proof of physical and proof of immunizations ? I contacted the family at 2pm and was provided the physical and shot record at 3:15pm

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not have documentation of a physical exam by the 30th day of attendance. Evidence: Child #3, enrolled 10/17/2019, did not have documentaion of a physical in the record.

Plan of Correction: Child#3 proof of physical and proof of immunizations ? I contacted the family at 2pm and was provided the physical and shot record at 3:15pm

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, not all records contained the required information. Evidence: Child #3, enrolled 10/17/2019, did not have documentation of proof of age/identity in the record. Child #5, enrolled 9/17/12, did not have documentation of a second emergency contact or documentation of parent confirmation of annual update of the child's records.

Plan of Correction: Child #3 proof of age ? I contacted the family at 2pm and dad provided a copy of the birth certificate at 3:15pm
Child#5 proof of 2nd emergency contact ? I contact her mom and was provided the name, address, and phone number of a second emergency contact.
