This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A Monitoring inspection was initiated on 7/01/2020, and concluded on 7/31/2020. The director was contacted by telephone to initiate the inspection. There was 38 children supervised by 8 direct care staff. The inspector emailed the director/ provider a list of items required to complete the inspection. The inspector reviewed 5 children?s records and 6 staff records submitted by the facility to ensure documentation was complete.

Evidence: Child #2 (date of enrollment 12/16/2019) did not have a physical examination within one month after attendance.Plan of Correction: Continue review of child files and ensure all have an updated physical within 1 year.
Standard #: 22VAC40-185-60-A Description: Based on children record review, 1 out 5 records, the center did not obtain and keep at the center in child's record documentation of viewing proof of the child's identity.
Evidence: Child #2 (date of enrollment 8/18/2017) did not have documentation of viewing proof of identity.Plan of Correction: Continue review of child's files and ensure we have all nec. paperwork before start date.
