Standard #: 22VAC40-191-40-D-1-A Description: Based on record review and interview, the center failed to ensure that a new person designated as applicant had a completed background check before the end of 30 days of becoming a Board officer (applicant).

Per the interview with the Program Director, Board officer #4 began his/her service as a board member on January 1, 2020 and did not have any of the three forms completed that comprise a background check (sworn statement, central registry searches, and the criminal records checks) until at least July of 2020.Plan of Correction: Not available online.
