Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on August 31, 2020 and concluded on August 31, 2020. The director was contacted by telephone to initiate the inspection. There were 62 children present and 13 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed four children?s records and four staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of four children's records, the center did not ensure that one child's record contained documentation of obtaining the child's immunization record prior to the first day of attendance. Evidence: The record for Child #3 (start date 7/18/19) contained an immunization record dated 7/29/19.

Plan of Correction: Administration will ensure that children's immunization records are obtained and documented prior to the first day of attendance.

Standard #: 22VAC40-185-70-A
Description: Based on a review of four staff records, the center did not ensure that one staff record contained documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: The record for Staff #4 (hire date 8/23/19) did not contain documentation of having obtained two references.

Plan of Correction: Administration will ensure that the staff file is updated and will ensure that references are documented in staff files prior to employment.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of four staff records, the center did not ensure that two staff records contained a central registry finding within 30 days of employment. Evidence: 1. The record for Staff #3 (hire date 10/12/16) did not contain a central registry finding. 2. The record for Staff #4 (hire date 8/23/19) contained a central registry finding dated 10/7/19.

Plan of Correction: Administration will ensure that background checks are obtained and documented in staff files within the required time frames. Administration will provide the licensing inspector with documentation showing the central registry search request for Staff #3 is mailed within 10 days.
