1. Reviewed supervision requirements with vendor. 2. Form provided: daily attendance.

Comments:
7/26/19 - 10:10-11:20am On-site to conduct an unannounced complaint investigation with a local agency into allegations of non-compliances in the program and supervision. Records reviewed, interviews conducted, and observations made. There was insufficient evidence to support program allegations however, there was a preponderance of evidence to support supervision of children and therefore that portion of the investigation has been deemed to be valid. Additional violations were found and cited in this report. Supervision findings were discussed with the vendor today. The vendor does not speak or read English, and a visiting adult family member was present and willing to translate between inspector and vendor. 8/6/19 - The inspection report was issued to the vendor. A written plan of correction for supervision (280.A) is due back to the inspector by 8/16/19.

Standard #: 22VAC40-665-160-B
Description: Based on records reviewed and interview conducted, it was determined that the vendor has not maintained a written hard copy of daily attendance that documents the arrival + departure of each child in care as it occurs. Evidence: The last date the written attendance was recorded was 1/30/19. The vendor said she has not kept written daily attendance.

Plan of Correction: The vendor will write down attendance everyday on paper.

Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 3 of 4 children's records did not include required information. Evidence: 1. Child #2 - Name, home/work address for 1 of 2 parents. 2. Child #3 - Name, home/work address for 1 of 2 parents. 3. Child #4 - Name, home/work address for 1 of 2 parents.

Plan of Correction: Child #2 and #3 are no longer enrolled. The vendor will request parent of Child #4 to complete required information.

Standard #: 22VAC40-665-280-A-1
Description: Based on interviews conducted, it was determined that the vendor was not aware of activities children were engaged in, and was not near enough to intervene. Evidence: 1. In May 2019, Child #3 took a photo on an electronic device of Child #2 using the bathroom, and proceeded to send it to his parent. 2. The vendor said she was not initially aware of the incident, but that it was brought to her attention by the parent of Child #4 at the time it occurred. Her response to the parents concerns of the photo being taken was "...I don't know...I'm sorry..." 3. Child #4, who was present during the May 2019 incident, said that he observed Child #3 take the photo of Child #2, who was sitting on the toilet. Child #4 described Child #3 as being "naked" at the time the photo was taken, and that the bathroom door was open. 4. Child #4 viewed the photo on Child #'3's electronic device, proceeded to delete the photo from the device. 5. The vendor said that the bathroom door is typically halfway open/closed when children use it. 6. The vendor was not able to tell the inspector where she was at the time the incident occurred.
