Technical Assistance:

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 09/16/2020 and concluded on 09/28/20 The owner was contacted by telephone to initiate the inspection. There were 3 children present and 2 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Violations:

Standard #:	22VAC40-185-60-A
Description:	Based on a record review the center did not ensure all required information was in each child's record. Evidence: Two of three children's records reviewed did not have all required information in the record. Evidence: Child #1 and child #2 (both enrolled 08/10/20) did not have 1 of the 2 required emergency contacts.
Plan of Correction:	The following corrective action was provided by the owner during the exit interview: The owner will obtain this information from the parents today at pick-up and document it in the record.
Standard #:	22VAC40-191-60-B
Description:	Based on a record review the center did not ensure all employees had a completed sworn statement of affirmation. Evidence: Staff #2 had a sworn statement in the file , however it was not a form that is used for licensed child day programs and did not include all the required information.
Plan of Correction:	The following corrective action was provided by the owner during the exit interview: This was an oversight. A new sworn statement will be completed on the correct document.
Standard #:	22VAC40-80-120-E-2
Description:	Based on a record review the center did not ensure the findings of the most recent inspection of the facility was posted. Evidence: The center did not have the last inspection findings from 06/18/20 posted in the building. The owner confirmed it was not posted.
Plan of Correction:	The following corrective action was provided by the owner during the exit interview: The owner will ensure the most recent inspection is posted in the center.
Standard #:	63.2(17)-1720.1-B-2
Description:	Based on a record review the center did not ensure all employees had fingerprinting. Evidence: Staff #1 did not have documentation of the results of a fingerprint. The fingerprint was done through an incorrect form and the results came back as if the center were a family day home.
Plan of Correction:	The following corrective action was provided by the owner during the exit interview: The staff went back on 09/11/20 to have this corrected and are waiting for the results. The owner will follow-up in the status and submit the results as soon as they are received.
