Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 9/15/2020 and concluded on 9/15/2020. The director was contacted by telephone to initiate the inspection. There were 43 children present and 10 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not all contain required information.

Evidence:
Child#1's record did not contain the father's work phone number. Child#3's record did not contain the parents' work phone numbers and the emergency contacts' home addresses.

Plan of Correction: Forms are being updated to make this information required.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, the center did not obtain a central registry finding for a staff member within 30 days of employment.

Evidence:
Staff#1's record did not contain a central registry finding. The staff member's first day of employment was documented as 8/29/2019.

Plan of Correction: A new form has been submitted.
