Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.


A complaint inspection was initiated on 9/30/2020 and concluded on 10/1/2020. A complaint incident was received by the department regarding allegations in the areas of procedures for emergencies. The director was contacted by telephone to conduct the investigation.


The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued.

Description: Based on record review and interviews conducted, parents were not notified by the end of the day for minor injury.
Evidence:
Child 1 (10 months of age) sustained a visible injury to the forehead on 9/29/2020. No member of the staff working at the center notified the parent of the nature of the child's injury prior to the child being picked up from the center. The parent noticed the injury upon pick up and proceeded to bring the injury to the administration's attention. There was not a written injury report available for the parent of the date of the injury.

Plan of Correction: Remind staff to write a incident report for all minor incident as well.

Description: Based on record review, the center did not maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence to include when the parent was notified.
Evidence:
An incident, report written on 9/30/2020, was reviewed for an incident occurring on 9/29/2020. The written report did not contain information of when the parent was notified of the incident.

Plan of Correction: Remind staff to make certain all parents notification are filled out.
