63.2(17) License & Registration Procedures
- Technical Assistance:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on August 26, 2020 and concluded on September 22, 2020. A complaint incident was received by the department on August 3, 2020 regarding allegations in the area of forbidden actions. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation did not support the complaint of non-compliance with standards or law. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Standard #: 63.2(17)-1720.1-B-2 Complaint related: No Description: Based on a review of records on 8/27/20 and interview on 9/22/20, the center did not ensure to obtain a fingerprint based national criminal record check prior to the first day of employment for each staff.
Evidence: 1. The record of Staff #2 contained documentation of fingerprints dated 9/27/18. Documentation of Staff # 2's orientation is dated 8/21/18.
2. The record of Staff #3 (DOH 9/23/19) contained documentation of fingerprints dated 3/30/18. Per administration, Staff #3 is a rehire and the last documentation of fingerprints is dated 3/30/18. Administration acknowledged the fingerprint check was not completed prior to rehiring Staff #3.
Plan of Correction: Kidzalat will ensure that a fingerprint national criminal record check has been obtained (performed) before the first day of employment.
