Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 10/01/2020 and concluded on 10/05/2020. The provider was contacted by telephone to initiate the inspection. There were 40 children present and 6 staff members present during the inspection. The inspector reviewed 3 children's records and 3 staff records during the inspection to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the provider.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and discussion with staff, the facility failed to ensure that staff records, containing all of the elements as required by the standards, shall be kept for each staff person .

Evidence: Staff #4 confirmed that there was not a staff record available for review for Staff #3. Staff #4 stated that Staff #3 has been working since 09/08/2020.

Plan of Correction: A complete staff record will be made for Staff #3.

Standard #: 22VAC40-191-60-B
Description: Based on record review and discussion with staff, the facility failed to ensure that an employee must not be employed until the agency has the person's completed sworn statement or affirmation.

Evidence: There was not a sworn statement or affirmation available for Staff #3; Staff #4 stated that Staff #3 has been working since 09/08/2020. Staff #3 was working during the inspection.

Plan of Correction: A sworn statement will be obtain.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and discuss with staff, the facility failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.

Evidence: There was not an employment eligibility letter for Staff #3. Staff #4 stated that Staff #3 has been working since 09/08/2020. Staff #3 was working during the inspection.

Plan of Correction: Staff #3 will get fingerprints done.
