Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on October 1, 2020 and concluded on October 2, 2020. The director was contacted by telephone to initiate the inspection. There were 6 children present and 3 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on observation and staff interviews, the licnesee did not have for each child enrolled a sperate record that contained all the required information.

Evidence:
The record for child #2 did not include information for the second parent name, home address, home phone number, employment location and employment phone.

Plan of Correction: Director will ensure that required information from the parents are completed where nothing is left blank.

Standard #: 22VAC40-185-550-D
Description: Based on documentation review and staff interviews, the licensee did not ensure that the center shall implement a monthly practice evacuation drill and a a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Evidence: The center director confirmed there was childcare in August and there was not a drill conducted in August.

Plan of Correction: Conduct a fire drill every month.
