Technical Assistance:
Technical assistance was provided regarding tuberculosis screening/testing timeframes and children's records. In addition, new requirements outlined under Senate Bill 668 were reviewed.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 10/7/2020. The director was contacted by telephone to initiate the inspection. There were 45 children present and 8 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Wednesday, 10/14/2020. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, 1 of 3 children's records reviewed did not contain documentation that the child has received the immunizations required by the State Board of Health prior to attending the center.

Evidence: Child #3 did not have an immunization record on file.

Plan of Correction: Obtaining same-day. Called grandmother to obtain. Deny entry until obtained.

Standard #: 22VAC40-185-140-A
Description: Based on record review, 2 of 3 children's records reviewed did not contain documentation of a physical examination within one month of child's first day of attendance.

Evidence: Child #1 (Start Date: 2/6/20) and Child #3 (Start Date: 1/27/20) did not have physical examinations on file.

Plan of Correction: Obtaining same-day. Called grandmother to obtain. Deny entry until obtained.

Standard #: 22VAC40-185-540-C
Description: Based on review of the first aid kit and staff interview, the first aid kit was not complete.

Evidence: The first aid kit did not contain tweezers or at least two triangular bandages.

Plan of Correction: Purchased supplies and now on site. Corrected.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center did not ensure all staff have submitted to a fingerprint based national criminal history background check prior to first day of employment.

Evidence: Staff #2 (Date of Hire: 9/4/20) had fingerprinting results on file dated 9/29/2020.

Plan of Correction: Ensure that FBI fingerprinting is complete before employment in the future. No exceptions.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review, the center did not obtain a sex offender registry check from all states in which staff members have resided within the last 5 years prior to employment.

Evidence: Staff #1 (Date of Hire: 8/28/2020) did not have a sex offender registry check on file for Minnesota and Florida. Staff #3 (Date of Hire: 8/10/2020) did not have a sex offender registry check on file for Florida.

Plan of Correction: Remain up-to-date with current legislation changes ongoing. Call licensing if any questions about changes.
