This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 4/22/20 and concluded on 7/29/20. An initial complaint was received by the department on 3/6/20 regarding allegations in the area of food services. Additional complaints were also received by the department regarding allegations in the areas of supervision and ratios. The allegations were investigated jointly with the local agency. The director was contacted by telephone to conduct the investigation. Interviews with staff and collaterals were conducted by telephone and documentation and records related the complaint were reviewed. Based on information gathered during the investigation, the portion of the complaint regarding supervision is determined to be valid and violations were issued in the area of staffing and supervision.

Standard #: 22VAC40-185-340-A Complaint related: No Description: Based on interviews, it was determined that the center did not ensure that when staff are supervising children, they shall always ensure their care and protection.
Evidence:
1. During the week of February 17, 2020, staff 1 observed child 1 (age 1 year) outside on the playground alone and unsupervised.
a. Staff 1 stated she was in her classroom preparing lunch when she saw child 1 through the window running back and forth outside on the playground.
b. Staff 1 stated she told staff 2 (child 1's teacher) that child 1 was outside on the playground and that staff 2 went outside and brought child 1 back inside to the classroom. Staff 1 stated staff 2 appeared to be "in shock" when she informed her that child 1 was alone on the playground.
2. Child 1's teachers (staff 2, staff 3, staff 4 and staff 5) all confirmed that they did not know child 1 was not in the classroom and did not know how child 1 ended up alone on the playground.Plan of Correction: The center responded with the following: Will provide plan of correction after desk review.
Standard #: 22VAC40-185-340-F Complaint related: No Description: Based on interviews, it was determined that the center did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
1. During the week of February 17, 2020 , staff 1 stated she observed child 1 (age 1 year) alone and unsupervised on the center's playground.
2. Staff members responsible for the direct care of child 1 confirmed that child 1 was found unsupervised on the playground. Child 1 was out of sight and sound supervision of staff for a minimum of approximately 10 minutes and a maximum of approximately 30 minutes.Plan of Correction: The center responded with the following: Will provide plan of correction after desk review.
