Discussion of incident that occurred on 10/05/2020, at approximately 9 am.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. The following inspection approximately took place between the hours of 11:30 am through 11:40 am. This unannounced Non-mandated Monitoring Inspection (NMI) was conducted as a result of an incident reported by the center to the Division of Licensing. Standard #: 22VAC40-185-40-E Description: Based on interview, staff did not ensure that the center's activities were maintained in compliance with the center's own policies and procedures that were required by these standards.
Evidence -
1. Administrative Staff stated (during an interview conducted on the date of inspection - 10/06/2020) that Staff #1 and Staff #2 failed to conduct a sweep of the playground, and a name to face count while transitioning their group of children from the Toddler playground back inside to their classroom, thus leaving (1) one child behind. This incident occurred on 10/05/2020 at approximately 9am.
2. Video recording revealed that Staff #1 and Staff #2 did not follow their training or center procedures on the maintaining of supervision for the children in care.
3. This incident was self-reported by the center to the Division of Licensing.Plan of Correction: This matter was handled administratively by the center. Further training on supervision shall be conducted for staff.
Standard #: 22VAC40-185-340-F Description: Based on interview, a child under the age of 10 was not within sight/sound supervision.
Evidence -
1. Administrative Staff stated (during an interview conducted on the date of inspection - 10/06/2020) that on 10/05/2020, at approximately 9am, Child #1 (2 years of age) was left outside on the Toddler playground, not within sight/sound supervision of Staff #1 and Staff #2.
2. After a couple of minutes, Staff #1 and Staff #2 realized that Child #1 was not inside and retrieved this child from the playground.
3. While Child #1 was not under their direct supervision, other center staff were also outside on an adjacent playground supervising their own classroom.Plan of Correction: This matter was handled administratively by the center. Further training on supervision shall be conducted for staff.
