Technical Assistance:
1. Reviewed bathroom supply requirements. 2. Reviewed/trained vendor on new subsidy requirements. 3. Forms provided: sick child policy, proof of identity, and new emergency plan.

Comments:
10:50am-12:05pm Annual unannounced subsidy inspection. There were no children present today, though there are currently 6 children enrolled. The home was found to be clean, safeguarded appropriately, and sufficiently supplied with age appropriate toys and games for the children. The vendor completed the annual child development training, and is currently certified in First Aid/CPR. Recordkeeping non-compliances found.

Description: Based on records reviewed, it was determined that 4 of 6 children's records did not include required information. Evidence: 1. Child #1 - Name, address + phone number of emergency contact person. 2. Child #2 - Name, address + phone number of emergency contact person. 3. Child #5 - Parents home/work addresses and phone numbers; address of emergency contact person. 4. Child #6 - Work address + phone number for 1 of 2 parents; address of emergency contact person; written authorization for emergency medical care.

Plan of Correction: Vendor will require parents to complete the required information.
