This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on October 6, 2020 and concluded on October 9, 2020. The program director was contacted by telephone to initiate the inspection. There were 22 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-130-A Description: Based on record review, the Licensee failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
EVIDENCE:
Licensing Inspector reviewed four (4) children records. Record for Child #2 and #4 did not include documentation of immunizations. Child #2 enrollment date was 8/20/20 and Child #4 enrollment date was 9/08/20.Plan of Correction: Site Director will obtain documentation of up-to-date immunizations from the parents.
