Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on October 8, 2020 and concluded on October 9, 2020. The director was contacted by telephone to initiate the inspection. There were five children present and two staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-240-A
Description: Based on record review and interview, staff did not receive required training by the end of their first day of assuming job responsibilities.

Evidence: Staff #1's record did not have written documentation for orientation available for review. Staff #1 stated that orientation was not completed.

Plan of Correction: Staff will complete required orientation and moving forward, all staff will complete required trainings by the end of the first day of assuming job responsibilities.

Standard #: 22VAC40-191-60-B
Description: Based on record review and interview, one staff member did not complete a Sworn Disclosure Statement prior to the first day of employment.

Evidence: Staff #1's (Date of hire, 08/05/2020) record did not have documentation of a Sworn Disclosure Statement (SDS) available for review. Staff #1 stated that a SDS document was not completed.

Plan of Correction: Staff completed a Sworn Disclosure Statement and put it in the record. Moving forward, all new staff will complete a Sworn Disclosure Form prior tot he first day of employment.

Standard #: 22VAC40-191-60-C-1
Description: Based on record review, one staff record did not include documentation of a completed criminal history record prior to employment.

Evidence: Staff #1's (Date of hire, 08/05/2020), record had documentation of a Criminal Background determination letter dated 08/18/2020.

Plan of Correction: All Fingerprint based background check results are required prior to employment. Moving forward, all new staff will have criminal history results prior to employment.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the facility did not obtain a central registry finding within 30 days of employment.

Evidence: Staff #1's (Date of hire, 08/05/2020) record contained a central registry finding dated 10/01/2020.

Plan of Correction: Search of the Central Registry (CPS) background results for Virginia are required within 30 days of employment. Moving forward, all new staff will complete a search on the first day of employment.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, one staff record did not contain a copy of results of an out of state central registry for an employee that resided outside the state of Virginia within five years.

Evidence: Staff # 1's record, (Date of hire, 08/05/2020) did not have documentation of an out of state central registry available for review. Interview with Staff #1 confirmed that a out of state search was not completed.

Plan of Correction: Staff will complete an out of state Central registry Search and place it in the mail today. Moving forward, new staff that have lived outside the state within the last 5 years will complete a Central Registry Search.
