Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on October 16, 2020 and concluded on October 16, 2020. The Director was contacted by telephone to initiate the inspection and information was requested and received by the end of the day. There were 17 children present and 5 staff. The Inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 1 new staff record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility or the program. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-185-60-A
Description: based on record review, it was determined the center's maintained record for child 3 did not contain the parents employment addresses or their employment's phone numbers.
Evidence:
1. Child 3's enrollment form submitted for review did not include written employment name, employment address or phone number for either parent listed on the enrollment form.

Plan of Correction: The child was enrolled pre March and enrolled back with us in September. We will contact parents again to complete all documents.

Standard #: 22VAC40-185-60-A-7
Description: Based on interview and record review it was determined the center did not obtain actions to take in an emergency allergic situation for child 3.
Evidence:
1. Director stated Child # 3 has a listed food allergy to milk, dairy, eggs but there is no medication for the child.
2. Upon record review, the parent had listed food allergies and did not state what emergency action should be taken in the event the child ingested any of the items. The record only stated what symptoms it could cause.

Plan of Correction: I have called the parent and requested a new medical form to clearly state what actions need to be taken in the event of an allergic reaction.
