63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 10/15/2020 and concluded on 10/16/2020. The director was contacted by telephone to initiate the inspection. There were 22 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of two staff records, the facility failed to document all required information for each staff file. Evidence: two phone references taken for Staff A did not have include the signature of the person making the call.

Plan of Correction: When references are taken over the phone, the person making the call will sign the document.

Standard #: 22VAC40-185-240-A
Description: Based on a review of two staff files and staff interview, the facility failed to ensure that each staff receive orientation training by the end of their first day of assuming job responsibilities.
Evidence: The orientation training for Staff B (date of hire 9/8/2020) was signed, but not dated.

Plan of Correction: Orientation will be signed and dated at the end of each employee's first day.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on review of two staff files, the facility failed to have documentation of a completed sworn disclosure statement for each staff prior to the first day of employment. Evidence: Staff B did not have documentation of a completed sworn disclosure statement. The sworn statement e-mailed to the licensing inspector on 10/16/2020 was not dated.

Plan of Correction: Employees will sign and date a sworn disclosure statement prior to the first day of employment.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of two staff files and staff interview, the facility failed to obtain a search of the central registry (CPS check) within 30 days of hire. Evidence: Staff B (date of hire 9/8/2020) did not have documentation of having submitted a search of the central registry (CPS check).

Plan of Correction: The search of the central registry (CPS check) was mailed 10/16/2020.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of two staff files and staff interview, the facility did not ensure that all staff submit to fingerprinting and obtain results prior to employment. Evidence: Staff B (date of hire 9/8/2020) did not have documentation of having submitted to fingerprinting and obtaining results prior to employment.

Plan of Correction: Staff B is scheduled to have fingerprints done 10/17/2020.
