Technical Assistance:

Reviewed all background requirements for agents and employees to include new background requirements, effective 7/01/2020, for individuals who currently reside outside of the state of Virginia or who have resided outside of the state of Virginia in the past 5 years.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 10/09/2020 and concluded on 10/09/2020. The director was contacted by telephone to initiate the inspection. There were 8 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records, and background checks for all agents submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-185-130-A
Description:	Based on record review, the center did not obtain documentation that one child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. Child #1 began attending the center on 8/31/2020. 2. Documentation of Immunizations in the record for Child #1 were dated 9/18/20.
Plan of Correction:	Immunizations were obtained and placed in the record.
Standard #:	63.2(17)-1720.1-B-4
Description:	Based on record review, the center did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Evidence: 1. Agent #1 (start date 8/26/20) has resided outside of the state of Virginia within the past 5 years. 2. A criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry were not obtained from the state of Arizona.
Plan of Correction:	The center will provide documentation that the requests were sent within 10 business days.

Description:	Based on record review, an out of state child abuse and neglect search was not completed for 1 staff. Evidence: 1. Documentation in the record for Staff #1 (hire date1/2/15) indicated Staff #1 resided outside of the state of Virginia within 5 years of the date the documentation was completed. No Out of State Child Abuse and Neglect search results were obtained from the state of Pennsylvania.
